Who we are
SLR Homecare Inc. provides personal support care in the home across the Greater Toronto Area. When we provide care, SLR Homecare Inc. acts as a health information custodian under Ontario's Personal Health Information Protection Act, 2004 (PHIPA). We also follow the federal Personal Information Protection and Electronic Documents Act (PIPEDA) for personal information generally.
Information you give us through our website
If you use our Request a free assessment form, we collect your name, phone number, email address (if you give one), your town or neighbourhood, the best time to call, and anything you choose to tell us about the help you're looking for. We use it only to call you back and talk about care.
Please don't include medical details in the form. We'll talk about those by phone. The form is sent over an encrypted connection and is seen only by the people at SLR who need it to respond to you.
If care does not go ahead, we keep your request for twelve months and then securely delete it. If care does go ahead, your information becomes part of your client record and is retained as required for health records in Ontario.
How we use your information
- To assess, plan, provide and coordinate your care.
- To communicate with you and, with your consent, your family or other care providers.
- To manage scheduling, billing and insurance, including WSIB and private insurers.
- To meet our legal and professional obligations.
We do not sell your information, and we do not share it for marketing. We share health information with others involved in your care only as needed and as the law allows.
Keeping your information safe
We protect your information with administrative, physical and technical safeguards appropriate to its sensitivity, and we limit access to the people who need it to do their jobs.
If a privacy breach occurs
If your personal health information is stolen or lost, or is used or disclosed without authority, we will notify you at the first reasonable opportunity. Where the law requires it, we will also notify the Information and Privacy Commissioner of Ontario.
Our notice will tell you what happened, what information was involved, what we are doing in response, and how to reach our Privacy Contact.
Your rights
You may ask to see the personal health information we hold about you, and ask us to correct it if it's wrong. You may withdraw your consent to certain uses, subject to legal and care limits. Contact our Privacy Contact below and we'll help.
If you email us at info@slrhomecare.com, we use your message only to reply to you and, if care goes ahead, to keep your client record complete. We never sell or share your email address.
Cookies and how the site is used
Our website does not use advertising or tracking cookies. Like most websites, our hosting provider automatically records basic technical information (IP address, browser and device type, the page that referred you, and the pages you visit) to keep the site working and secure, not to identify you personally. Our pages also load typefaces from Google Fonts, which receives your browser's IP address when the page loads.
If you apply for a job with us
When you apply to work with SLR Homecare, whether through careers@slrhomecare.com or any form on this site, we collect the information you send us: your resume and work history, your PSW certification and any other credentials, references, and your contact details.
We use it only to assess your application, contact you, and verify your credentials and police record check where the role requires one. If we do not hire you, we keep your application for twelve months in case a suitable role comes up, then securely delete it. We do not share your application with anyone outside SLR Homecare without your consent.
Links to other websites
This site may link to other organisations, such as government programs, community services and insurers. Those sites have their own privacy policies and we are not responsible for how they handle your information. Please read their policies before giving them anything personal.
Children
This website is not directed at children, and we do not knowingly collect information directly from anyone under 13 through it. Where care involves a minor, information is provided by a parent, guardian or other lawful
Where your information is kept
Your information is stored in Canada wherever possible. Our website is hosted by Netlify, and requests sent through our form are stored on Netlify's servers, which may be outside Canada, including in the United States. Some of our other service providers, for example for email, may also store or process information outside Canada, in which case it may be subject to the laws of that country. We choose providers who are contractually required to protect your information and use it only for the purpose we have given it to them.

SLR Homecare Inc. has designated a Privacy Contact responsible for our compliance with this policy and with PHIPA. You may contact them with any question, concern or complaint about how your personal health information is handled, or to request access to or correction of your records.
